What to Include in Notes
- Specific activities affected by pain
- What makes pain better or worse
- Medications taken and their effects
- Sleep quality and issues
- Emotional impact (frustration, anxiety)
Examples of Helpful Notes
Couldn't carry groceries from car to house - had to make 6 trips. Right shoulder pain 7/10 when lifting.
Woke up 3 times from neck pain. Only slept about 4 hours total. Took Tylenol at 2am.
Had to leave work early because headache got so bad I couldn't focus on computer screen.
Good day - pain at 3/10. Was able to walk around the block for the first time since accident.
Be Honest and Specific
- Record both good and bad days
- Use specific numbers when possible
- Describe what you couldn't do
- Note any new symptoms
When to Add Notes
Log notes:
- At the end of each day
- When something significant happens
- After any activity that affects your pain
- When you notice changes in symptoms
