How to Use the Case Journal

Everyday Challenges

Use this space to describe how normal tasks have become harder or things you can’t do anymore.

Examples include:

  • Having trouble working a full day.
  • Struggling with chores like cooking, cleaning, or laundry.
  • Difficulty caring for kids or pets.
  • Feeling unsafe or uncomfortable while driving.

Things You Miss

Share the activities, hobbies, or social time you’ve had to cut back on or stop altogether.

Examples include:

  • Missing hobbies like sports, gardening, or crafts.
  • Spending less time with friends or family.
  • No longer being able to walk, bike, or travel the way you used to.
